Requirements
For non-Real ID
driver licenses, permits
and ID cards
Full legal name
Your full legal name is the name recorded on
your document that proves identity. If the
name on your proof of identity is dierent
than your current name, you must show DMV
additional proof of identity or legal name change
documents proving your current name.
Examples of legal name change:
Government-issued marriage certicate/license,
divorce decree, judgment of dissolution or
annulment of marriage or domestic partnership,
Oregon certicate of registered domestic
partnership, adoption decree, or a court order.
Social Security Number
If you have been assigned a Social Security
number, you must provide it in order to be
issued a driver license, permit or ID card. If you
don’t have a Social Security number, when you
apply for your driver license, permit or ID card
you will be asked to sign an electronic statement
certifying that you haven’t been assigned a Social
Security number.
Don’t use false ID documents
All documents that prov
e full legal name,
identity and date of birth must be original
or certied copies from the issuing
agency. DMV has the discretion to require
additional evidence of full legal name, Social
Security number, identity and date of birth.
It is a crime to present altered or forged
documents or to make a false statement to
DMV when applying for a driver license,
permit or ID card, and is punishable by a
ne and up to a year in prison (ORS: 25.785,
807.530, 809.310).
Frequent questions
I previously had an Oregon driver license, but
it has been expired for over two years. Am I
able to renew it?
No. If your previous Oregon license has been
expired for over two years, you will need to
apply for an original Oregon driver license.
You will need to take all required tests, and
pay original issuance fees. DMV will use your
previous driver license number.
I want to get a Real ID driver license to
use to board ights within the U.S. Are the
requirements dierent?
Yes. To qualify for a Real ID driver license,
permit or ID card you must present proof of
lawful status, as well as other documents. Visit
www.Oregon.Gov/RealID for full requirements.

ID Requirements
Applicants for new, renewal or replacement
driver licenses, instruction permits or
identication cards must provide acceptable
proof of identity, current full legal name and
date of birth.
If you are unable to meet
these requirements
DMV cannot issue a driver license, permit or ID
card until proof of identity and current full legal
name has been provided and your Social Security
number has been veried with the Social Security
Administration if you have been assigned one.
Other requirements
The identication requirements in this brochure
are in addition to payment of test and application
fees, proof of residence in Oregon, and passage
of the knowledge, vision and drive tests.
Proof of identity, current full legal
name and date of birth, includes:
• One Primary Identity Document; or
• Two Secondary Identity Documents; or
• One Secondary Identity Document and DMV
Photo on le less than 13 years old
All required and supporting documents
MUST be the original or a certied copy from
the issuing agency. Photocopies of identity
documents are not acceptable.
Primary Identity Documents include:
• U.S. or Canadian government-issued birth
certicate (hospital birth certicates are not
acceptable)
U.S. Consular Report of Birth Abroad (FS-240)
Certication of Report of Birth (DS-1350 or
FS-545)
Request for Verication of Birth (DD372)
• Valid U.S. passport, U.S. passport card, or U.S.
Territory passport, not expired more than ve
years and cannot have clipped corners or be
hole-punched
• Tribal ID Card from a federally recognized
tribe located in Oregon, or with an Oregon
aliation, approved by DMV
Foreign Passport containing English or with
English translation, not expired more than

IRS (Must include name and date of birth)
• A Consulate ID Card, approved by DMV, not
expired more than five years
Secondary Identity Documents include:
• U.S. Military discharge papers
• An Oregon student body card
U.S. or foreign certied school records
A W2 or 1099 tax form for the current tax year
• A Medicare Card
• A court document issued by a court in the
U.S. that shows the applicant is a party to a
judicial proceeding and which contains court
signatures and seals. Acceptable documents
are:
• Gender, name change, or both
• Adoption, guardianship, custody, or child
support
• Judgment of dissolution or annulment of
marriage
• U.S. government issued marriage license,
domestic partnership, or Certicate of Record
of Marriage
Birth certicate, marriage license, driver
license, military ID card, passport or voter
card issued by a foreign government
A benets leer issued by the Social Security
Administration
Veterans’ Universal Access Identication Card
A leer issued by the Oregon Health Plan, if
dated within the last year
U.S. Tribal Identication Card
Identication Card that contain photograph of
the applicant issued by a government within
the U.S. or the U.S. government
