ID Requirements
Applicants for new, renewal or replacement
driver licenses, instruction permits or
identication cards must provide acceptable
proof of identity, current full legal name and
date of birth.
If you are unable to meet
these requirements
DMV cannot issue a driver license, permit or ID
card until proof of identity and current full legal
name has been provided and your Social Security
number has been veried with the Social Security
Administration if you have been assigned one.
Other requirements
The identication requirements in this brochure
are in addition to payment of test and application
fees, proof of residence in Oregon, and passage
of the knowledge, vision and drive tests.
Proof of identity, current full legal
name and date of birth, includes:
• One Primary Identity Document; or
• Two Secondary Identity Documents; or
• One Secondary Identity Document and DMV
Photo on le less than 13 years old
All required and supporting documents
MUST be the original or a certied copy from
the issuing agency. Photocopies of identity
documents are not acceptable.
Primary Identity Documents include:
• U.S. or Canadian government-issued birth
certicate (hospital birth certicates are not
acceptable)
• U.S. Consular Report of Birth Abroad (FS-240)
• Certication of Report of Birth (DS-1350 or
FS-545)
• Request for Verication of Birth (DD372)
• Valid U.S. passport, U.S. passport card, or U.S.
Territory passport, not expired more than ve
years and cannot have clipped corners or be
hole-punched
• Tribal ID Card from a federally recognized
tribe located in Oregon, or with an Oregon
aliation, approved by DMV
• Foreign Passport containing English or with
English translation, not expired more than
ve years
• Certicate of Citizenship (N560 and N561)
• Certicate of Naturalization (N550, N570, and
N578)
• U.S. immigration or naturalization
documents, not expired more than ve years,
including:
• Resident Alien Card or Permanent
Resident Card (I551)
• Re-entry Permit Form (I327)
• Refugee Travel Document Form (I157)
• Employment Authorization Document
(I688A, I688B, I766)
• Military or Armed Forces ID Card
• Military Common Access Card
• U.S. Uniform Services ID and Privileges Card
(DD1173, DD1173-1)
• Out-of-state, District of Columbia, U.S.
Territorial government or Canadian driver
license, instruction permit, or ID card, with
photograph, not expired more than one year
unless hole-punched or marked “Not Valid as
ID”
• Oregon driver license, instruction permit, or
ID card, with photograph, not expired more
than thirteen years
• Non-immigrant visa issued by the U.S.
Department of State, not expired more than
ve years
• Temporary immigrant visa (I551) issued by
the U.S. Department of State, not expired
more than ve years
• Oregon Concealed Weapon Permit/Concealed
Handgun License, not expired more than ve
years
• Social Security Card or Individual Taxpayer
Identification Number issuance letter from the
IRS (Must include name and date of birth)
• A Consulate ID Card, approved by DMV, not
expired more than five years
Secondary Identity Documents include:
• U.S. Military discharge papers
• An Oregon student body card
• U.S. or foreign certied school records
• A W2 or 1099 tax form for the current tax year
• A Medicare Card
• A court document issued by a court in the
U.S. that shows the applicant is a party to a
judicial proceeding and which contains court
signatures and seals. Acceptable documents
are:
• Gender, name change, or both
• Adoption, guardianship, custody, or child
support
• Judgment of dissolution or annulment of
marriage
• U.S. government issued marriage license,
domestic partnership, or Certicate of Record
of Marriage
• Birth certicate, marriage license, driver
license, military ID card, passport or voter
card issued by a foreign government
• A benets leer issued by the Social Security
Administration
• Veterans’ Universal Access Identication Card
• A leer issued by the Oregon Health Plan, if
dated within the last year
• U.S. Tribal Identication Card
• Identication Card that contain photograph of
the applicant issued by a government within
the U.S. or the U.S. government
